Per Fresno Unified's policy for reopening schools for athletics, all student-athletes who plan on participating in sports for the 2020-2021 school year need to complete the self-screening tool DAILY prior to leaving their homes for practices. These electronic, sport-specific questionnaires have been created to help keep track of those daily answers, assist in recording daily attendance and be of aid in any future contract-tracing efforts that may arise. Please make sure to:

1. Click on the link of your current sport's questionnaire link
2. Use your Fresno Unified student email to sign-in and fill out your respective team's questionnaires

***questionnaires need to be completed PRIOR to leaving your home for practice***
***coaches will be using these questionnaires to take daily attendance of practices***​
